Prior to participating in services provided by Yoga Life with Amy, LLC, please review the information contained in the Disclaimer.**

LIABILITY WAIVER, RELEASE, AND COVENANT NOT TO SUE

1. I recognize that all activities related to yoga, workshops, retreats and/or trips involve physical activities which may be strenuous and may cause injury. I understand that I must judge my own capabilities with respect to any activity. By my participation in any activity or practice taught at or by Yoga Life with Amy, LLC, its employees, agents, instructors, and volunteers in any location, I assume full responsibility for any and all risks, injuries or damages, known or unknown, that I may incur.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any activity, and that no medical professional has advised me to refrain from participating in the activities or services to be provided. I represent and warrant that I am physically and mentally fit and have no medical condition which would prevent my full participation in any activity. I acknowledge that it is my responsibility to (i) inform the instructor of any injury or other physical or mental condition that might affect my ability to participate in any activities or movements, (ii) inform the instructor of any such injuries or conditions prior to engaging in activities or movements, and (iii) inform the instructor immediately if an injury occurs. BY PARTICIPATING IN ACTIVITIES AND MOVEMENTS FOR WHICH I AM BEING INSTRUCTED, I HEREBY COVENANT AND WARRANT THAT I DO NOT HAVE ANY PHYSICAL OR MENTAL CONDITION, LIMITATION, OR DISABILITY THAT WOULD LIMIT OR PRECLUDE MY PARTICIPATION IN SUCH ACTIVITIES, EXERCISES, OR PRACTICES, AND I ASSUME ALL RISKS.

3. Although I acknowledge there is no obligation whatsoever for any person to provide medical care during, prior to or after any activity related to Yoga Life with Amy, LLC, I hereby give permission for Yoga Life with Amy, LLC, its employees, agents, instructors, and volunteers to provide first aid and/or aid in seeking emergency medical treatment as they, in their sole and absolute discretion, may deem to be advisable on my behalf. IN THE EVENT SUCH TREATMENT IS PROVIDED TO ME, I HEREBY AGREE TO WAIVE, INDEMNIFY, HOLD HARMLESS, RELEASE AND DISCHARGE YOGA LIFE WITH AMY, LLC, AMY BISHOP, THEIR EMPLOYEES, AGENTS, INSTRUCTORS, OR VOLUNTEERS FOR, FROM, AND AGAINST, ANY CLAIM, INJURY, DAMAGES OR DEATH ARISING FROM THE PROVISION OF SUCH TREATMENT.

4. I understand that my participation in the activities and services provided by Yoga Life with Amy, LLC, is wholly voluntary. ACCORDINGLY, AND BINDING MY HEIRS AND LEGAL REPRESENTATIVES, I KNOWINGLY, VOLUNTARILY AND EXPRESSLY RELEASE, WAIVE, DISCHARGE, AGREE TO HOLD HARMLESS, AND COVENANT NOT TO SUE OR TO ASSERT ANY CLAIM AGAINST YOGA LIFE WITH AMY, LLC, AMY BISHOP, THEIR EMPLOYEES, AGENTS, INSTRUCTORS, OR VOLUNTEERS FOR, FROM, AND AGAINST, ANY CLAIM, INJURY, DAMAGES OR DEATH WHICH ARE RELATED TO, ARISE OUT OF, OR IN ANY WAY ARE CONNECTED TO MY PARTICIPATION IN THE ACTIVITIES AND SERVICES PROVIDED TO ME OF ANY KIND.

5. I acknowledge that Yoga Life with Amy, LLC, its employees, agents, instructors, or volunteers shall not be responsible or incur any liability for errors in data entry and documentation associated with my participation in the activities and services provided. I further understand that my contact information will be used for yoga studio related marketing purposes, but that I may opt-out or unsubscribe at any time.

6. I hereby certify that I am over 18 years of age, have carefully read the foregoing waivers, agreements, and the covenant not to sue, and I further acknowledge that I understand and agree to all terms and conditions contained herein above. Prior to signing this agreement, I acknowledge I had full and open opportunity to ask questions and to negotiate the terms so stated. I am aware that by signing this agreement, I assume all risks and potential injury or illness arising from my participation, and I waive and release all substantial rights that I may have and possess under the law as more fully detailed herein above.
